The Special Services Office coordinates tutoring services to students who are unable to attend school for an estimated period of time between four and eighteen weeks. Absences may be continual or intermittent. The district has forms that need to be completed and signed by a qualified medical practitioner that includes a diagnosis, and an estimate of the number of weeks the student will be out of school. A qualified medical practitioner is a doctor of medicine (M.D.), osteopathy (D.O.), naturopathy (N.D.), dentistry (D.M.D.), chiropractic (D.C.), or physician assistant (P.A.); an advanced registered nurse practitioner (A.R.N.P.); or a licensed mental health therapist. The form is returned to the Special Services Office for approval, processing, and assignment of a tutor.
To obtain the Home-Hospital forms please contact your child’s building school counselor or call the Special Services Office at (360) 277-2111.
